The Susan McLane Memory Wellness Center is a valuable resource for people suffering from memory loss and dementia. Run by Riverbend Community Mental Health, Inc. – in collaboration with Concord Hospital – and The Mental Health Center of Greater Manchester, along with Easter Seals NH, memory health services are offered in two communities – Concord and Manchester.

This center’s staff work with individuals and their families/caregivers, offering a range of services in an outpatient setting, including:

• Short-term counseling
• Support
• Education
• Referrals to ensure coordinated care
