The Susan McLane Memory Wellness Center is a resource for support and information. Clients typically are referred to the center by their primary care physician. Our staff members see clients and their family/caregiver for an assessment and then share their findings with the client and their family/caregiver. The program will integrate services and form relationships with the clients’ physicians.

This program will be a resource in an area where there is limited access to proactive assessment, treatment, education and advocacy for dementia and other memory problems. The Memory Wellness Center is a collaborative effort between Riverbend Community Mental Health Center and Concord Hospital in the Concord area and The Mental Health Center of Greater Manchester and Easter Seals - NH in Manchester. All the partners also look forward to working cooperatively with other community resources.
